Department-specific forms are completed by the student, then submitted to Patricia Jacobus in the Department of Sociology for processing. University e-forms are generally completed in Axess.

- Enrollment change petitions
- Application for extension of candidacy or MA program
- Graduation quarter petition
- Leave of absence request
- Permanently withdraw from degree program

- Office of the Vice Provost for Graduate Education: Always a great place to start, the website provides links to most university-wide resources related to graduate study at Stanford.

- Haas Center for Public Service: Stanford University's most visible commitment to public and community service.
